There used to be lots of disagreement over whether surgical interventions or radiation therapy was better for squamos cell carcinoma (SCC), but today there is consensus that while surgery probably delivers the highest cure rate in SCC, radiation therapy is itself very effective in bringing about cures in SCC.
Radiation is typically reserved for patients over age 60 because of the concerns about so-called long-term sequelae, or effects.
Either way, the surgeon is following the established treatment guidelines for SCC published by the National Comprehensive Cancer Network, among the most reliable in the world.
The mention of chemo must refer to a topical treatment. Alone, topical therapies aren't that great against SCC. In combination, I'm not sure.
At any rate, normally surgeons are willing to perform surgery on everyone, so it's nice to read that this surgeon is recommending the best known treatment for the patient, at least based on the factorsyou listed--- radiation therapy. Cure rates are very high for this treatment for this cancer type.
